Transition Services

Our interest as educators is to best prepare students to succeed in the endeavors they choose after completing high school. Our aim is for each student to live, learn, and earn as independently and successfully as possible upon graduation or exiting school. The goals of the transition planning process are to:

  • Incorporate a student’s needs, abilities, interests, and strengths into planning for postsecondary activities, including college and career readiness
  • Enable each student and their family to make informed choices
  • Include community experiences in school instruction
  • Develop a student’s employment and adult living skills
  • Enable each student to find success in moving from school to post-school activities

The Transition Department does not only work with our older students, but can support you as you apply for services outside of the school. We highly recommend beginning this process as early as possible to take advantage of the number of supports available to you.

Applying for OPWDD:

Through OPWDD, you can access supports and services for your student that can help your family to support them to live in your family home; assistance for them to live on their own in the community of their choice; help if they want to work, take part in volunteer opportunities or other types of community involvement; and assistance accessing services they need to live their life, including connecting them to clinical or therapeutic supports. These services are incredibly important for your child as they begin to transition out of our program and the process can be lengthy.

The first steps to becoming OPWDD eligible are contacting a Care Coordination Organization and completing what is called the Front Door workshop. In Brooklyn there are three care coordination organizations available to you:

  • Advanced Care Alliance of NYC Inc. (833) 692-2269
  • Care Design NY (518) 235-1888
  • Tri County Care (844) 504-8400 (currently not accepting new applications)

There are a number of documents we will need to collect in order to submit the packet for approval including evaluations that I will request. If you would like us to submit a request for evaluations on your behalf, please reach out to Miranda Young for support at myoung28@schools.nyc.gov

Documents we will need to collect:

  • Psychological evaluation
  • Psychosocial evaluations
  • Autism Evaluation (if applicable)
  • Annual Physical Evaluation (from your child’s primary care doctor)
  • IEP
  • Insurance Cards
  • Birth Certificate or State ID
  • Social Security Card

Other documents are sometimes requested by OPWDD, I will work with you and your selected CCO in order to ensure your packet is completed and we stay on track. If you have any questions at all throughout the process, always feel free to reach out.
